Understanding All About Pregnancy Complications

Various Pregnancy Complications:

Pregnancy is a physiological state of the human body. Hence the major responsibility is of the obstetrician providing antenatal care .

Antenatal Pregnancy Complications:

Identifying high risk factors based on history, examination and investigation results. Planning of pregnancy to suit individual requirements.

Pregnancy complications are divided into three types:

Antenatal Period

– Upto 12 weeks

– 13 weeks to 28 weeks

– 29 weeks to till labor

Intranatal period

– During delivery

Postnatal Period

– 6 weeks after delivery

Confirmation of pregnancy to 12 weeks.

  • Excessive vomiting that interfere with daily activities.
  • Vaginal bleeding /spotting – Abortion.

Basic history & examination & investigations to diagnose:

  • HTN (Hyper tension)
  • DM (Diabetes mellitus)
  • Hypothyroidism / Hyperthyroidism
  • Anemia
  • Urinary tract infection
  • Congenital malformations

Urinary Tract Infection

13 weeks to 28 weeks:

  • Vaginal bleeding
  • Leaking of fluid from vagina (Pre mature rupture of membrane)
  • Preterm delivery
  • Sudden intrauterine death
  • Gestational diabetes mellitus
  • Elevated blood pressure
  • Congenital fetal malformations (Diagnosed in ultrasound)

29 weeks to till labor:

  • Same as like (13 weeks to 28 weeks)
  • Postdated pregnancy
  • Decreased liquid (Amniotic fluid)
  • Low birth weight babies

Low Birth Weight Babies

Intranatal Period(during delivery): → Intrapartum  – 1st stage  – 2nd Stage  – 3rd stage

First stage:

  • Onset of labor pains to full dilation of cervix
  • Prolonged labor
  • Failure of descent of head
  • Failure of dilation of cervix
  • Big baby(Weight>4.5 kg)
  • Fetal distress

Second stage : 

  • Full dilation of cervix to delivery of the baby
  • Failure of descent of head
  • Shoulder dystocia
  • Still birth
  • Birth asphyxia
  • Delayed cry

Intranatal Period - Second Stage

Third stage:

  • Delivery of the placenta
  • Retained placenta
  • Postpartum haemorrhage
  • Vaginal & cervical lacerations

Postpartum period:

  • Urinary tract infection
  • Breast engorgement
  • Mastitis
  • Breast abscess
  • Wound infection
  • Episiotomy wound
  • Caesarean section wound
